We ask for your date of birth for identification purposes.
Our lawful basis for collecting Health Details is ‘contract’. Our obligation is to provide you with safe, appropriate exercises, in a class suitable for you, and we are only able to do this when we have up to date information regarding your health and well being, enabling us to manage risk and safety issues inherent in all exercise systems. This health information is shared within the contracted teaching team at Move. Studio. If your health changes in any way you will need to let us know, so we can continue to teach you safely. To preserve your privacy, please either email or telephone your teacher outside of the class situation, or arrive at class early so you can discuss the matter in confidence with your class teacher before the scheduled class start time, outside of the class room if appropriate.
We are lawfully required to keep your health data for at least 7 years after you finish taking class with us. Following this, paper work is shredded and electronic data is disposed of securely.
How we collect data
The data is collected via telephone, email and text enquiries, online booking forms, through the completion of health forms and occasionally verbally in class. (Please ensure you arrive early to class if you need to discuss confidential health issues with your teacher so we can give you the time and privacy to do this.)
